As an older adult, being independent has always been a source of pride
for you. Part of that independence is living at home, in your community,

But sometimes you need assistance with your health care, transportation,

This is where the Program of All-Inclusive Care for the Elderly, or PACE,
can help.
PACE allows senior citizens who would otherwise require nursing
home care to live in their homes as long as possible by providing:
Transportation to/from the PACE center & other medical appointments.
A team of healthcare professionals. Social Activities.
Primary and specialist care. Medication Management.
Assistance at home with daily activities. And much more.
